Covid-19 : l’OMS demande la levée des interdictions de voyage et du passe vaccinal pour voyager
Alors que les cas d’Omicron continuent d’augmenter dans le monde, l’Organisation mondiale de la Santé (OMS) s’est prononcée contre l’interdiction des voyages internationaux et la vaccination obligatoire pour entrer dans les pays, invoquant l’inefficacité de telles mesures pour enrayer la propagation du variant Omicron.
Le Comité d’urgence du Règlement sanitaire international (2005) de l’OMS a émis ces recommandations à l’issue de sa dixième réunion sur la Covid-19 qui s’est tenue mercredi 13 janvier 2022. Il est notamment recommandé de « lever ou d’assouplir les interdictions de circulation internationale, car elles n’apportent aucune valeur ajoutée et continuent de contribuer au stress économique et social des États parties », a dit le Comité, qui estime que la pandémie reste assez grave pour mériter le plus haut niveau d’alerte de l’OMS.
Selon l’agence sanitaire mondiale des Nations Unies, la mise en œuvre d’interdictions générales de voyager « n’est pas efficace pour supprimer la propagation internationale » et « peut décourager la notification transparente et rapide des variants émergents préoccupants ».
Une façon pour les experts de l’OMS de rappeler que « les mesures de voyage telles que le masquage, le dépistage, l’isolement/la quarantaine et la vaccination doivent être fondées sur des évaluations des risques et éviter de faire peser la charge financière sur les voyageurs internationaux, conformément à l’article 40 du Règlement sanitaire international (RSI) ».
Privilégier une approche fondée sur le risque pour faciliter les voyages internationaux
Selon des conclusions publiées hier mercredi, l’OMS a également indiqué que l’obligation de fournir une preuve de vaccination contre la Covid-19 pour les voyages internationaux pourrait ne pas être nécessaire en tant que seule voie ou condition permettant les voyages internationaux. Il s’agit en fait d’une mesure renouvelée et déjà recommandée lors de la neuvième réunion du comité d’urgence.
« Ne pas exiger une preuve de vaccination contre la Covid-19 pour les voyages internationaux comme seule voie ou condition permettant les voyages internationaux étant donné l’accès mondial limité et la distribution inéquitable des vaccins contre la Covid-19 », ajoute le Comité, invitant les États parties à envisager « une approche fondée sur le risque pour faciliter les voyages internationaux en levant ou en modifiant les mesures, telles que les exigences en matière de tests et/ou de quarantaine, le cas échéant, conformément aux directives de l’OMS ».
C’est dans ce contexte que le Comité a également modifié l’une de ses recommandations relatives à la campagne vaccinale dans le monde. Les experts défendent ainsi l’approche de l’OMS, qui invite la communauté internationale à faire en sorte qu’au moins « 70% de la population de tous les pays soit vaccinée au début de juillet 2022 et intégrer la vaccination contre la Covid-19 dans les services de santé de routine ».
L’OMS maintient l’alerte sur l’« urgence de santé publique de portée internationale »
A ce sujet, il s’agit de « reconnaître tous les vaccins ayant reçu la liste d’utilisation d’urgence de l’OMS et toutes les combinaisons vaccinales hétérologues conformément aux recommandations du Groupe consultatif stratégique d’experts de l’OMS sur la vaccination (SAGE), y compris dans le contexte des voyages internationaux ». Il est également demandé aux Etats parties de soutenir la recherche afin de déduire la stratégie de vaccination optimale pour réduire l’infection, la morbidité et la mortalité dans le monde.
La pandémie constitue toujours un événement extraordinaire qui continue de nuire à la santé des populations du monde entier, présente un risque de propagation internationale et d’interférence avec le trafic international, et requiert une réponse internationale coordonnée
Plus de 9,5 milliards de doses de vaccin ont été administrées dans le monde, selon un décompte établi par l’OMS le 19 janvier 2022.
Le Comité d’urgence de l’OMS, qui est présidé par le professeur français Didier Houssin, s’est réuni le 13 janvier, pour la dixième fois depuis le début de la pandémie.
C’est à la suite des recommandations de ce comité que le Directeur général de l’OMS, Tedros Adhanom Ghebreyesus, avait déclaré le 30 janvier 2020 que la Covid-19 constituait « une urgence de santé publique de portée internationale », le plus haut degré d’alerte de l’OMS.
Dans ses conclusions, le Comité d’urgence indique avoir convenu la semaine dernière que ce niveau d’alerte reste justifié. Le comité a convenu « à l’unanimité » que la pandémie « constitue toujours un événement extraordinaire qui continue de nuire à la santé des populations du monde entier, présente un risque de propagation internationale et d’interférence avec le trafic international, et requiert une réponse internationale coordonnée ».
L’OMS souhaite une surveillance en temps réel des animaux réservoirs
Par ailleurs, le Comité d’urgence de l’OMS sur la Covid-19 a émis une nouvelle recommandation et la seule nouveauté de la réunion du 13 janvier dernier, qui demande aux pays de mettre en place une « surveillance en temps réel » des possibles animaux réservoirs du virus donnant la maladie. Il s’agit exactement de « mener des enquêtes épidémiologiques sur la transmission du SRAS-CoV-2 à l’interface homme-animal et une surveillance ciblée sur les hôtes et réservoirs animaux potentiels ».
« La surveillance en temps réel et le partage des données sur l’infection, la transmission et l’évolution du SRAS-CoV-2 chez les animaux permettront de mieux comprendre l’épidémiologie et l’écologie du virus », a dit le comité, qui estime que la pandémie reste assez grave pour mériter le plus haut niveau d’alerte de l’OMS. Cette surveillance permettra également d’identifier en temps nécessaire les éventuelles apparitions et évolutions de variants chez ces animaux et de pouvoir ainsi évaluer les risques en matière de santé publique, a indiqué le Comité.
Dans ses recommandations, les experts du Comité demandent donc aux États membres de l’OMS d’« établir des enquêtes épidémiologiques sur la transmission du SRAS-CoV-2 au niveau de l’interface homme-animal et une surveillance ciblée sur les hôtes et réservoirs animaux potentiels ».

Ces conseils semblent soutenir les recommandations de l’Organisation mondiale de la santé animale (OIE), qui exhortait la semaine dernière les pays à « une surveillance ciblée des hôtes potentiels et le suivi continu des mutations du SRAS-Cov-2 chez les animaux.
« L’étude épidémiologique de la transmission du SRAS-CoV-2 à/par des animaux exposés à des patients atteints de Covid-19, la surveillance ciblée des hôtes potentiels et le suivi continu des mutations du SRAS-Cov-2 chez les animaux nous aideront à comprendre comment les virus évoluent chez les animaux, si de nouveaux variants apparaissent et si elles représentent une menace pour la santé publique », a souligné l’OIE dans un communiqué le 6 janvier dernier.
Pour cette organisation, il est essentiel que « les séquences animales et humaines, associées aux données épidémiologiques, soient partagées à l’échelle mondiale en temps réel ».
D’autant que selon l’OIE, « il a été démontré que le SRAS-CoV-2 peut infecter un large éventail d’espèces et le vison et le cerf de Virginie ont été identifiés comme des hôtes réservoirs potentiels ».
Récemment, on a beaucoup spéculé sur le fait que le variant Omicron, « qui présente une constellation inhabituelle de mutations, pourrait avoir évolué indépendamment chez un hôte animal ».
Plus de 332 millions de cas dans le monde dont plus de 5,5 millions de morts
Dans ces conditions, « la possibilité qu’Omicron ait émergé d’un hôte animal ne peut être écartée comme alternative à d’autres hypothèses, notamment l’évolution virale au cours d’une infection persistante chez une personne immunodéprimée ».
Il semble qu’il n’y ait pas de mutations partagées par Omicron et les virus d’origine animale qui n’aient pas été identifiées dans les virus humains.
« Toutefois, certaines mutations du pic d’Omicron sont associées à une adaptation chez la souris et, sur la base d’études in silico, un groupe de recherche a suggéré qu’Omicron pourrait avoir évolué chez la souris avant de se répandre chez l’homme », a dit l’OIE dans un communiqué.
Ces recommandations du Comité d’urgence de l’OMS et du Groupe consultatif de l’OIE sur l’évolution du SRAS-CoV-2 chez l’animal interviennent alors que des records de contamination sont notés dans plusieurs pays avec le déferlement d’Omicron. Plus de 900.000 cas de Covid-19 en moyenne ont été détectés chaque jour entre le 22 et le 28 décembre, un nombre encore jamais atteint depuis le début de la pandémie fin 2019, qui représente une hausse de plus de 35% par rapport à la semaine précédente.
Dans le monde, plus de 332,6 millions de cas ont été recensés dans 197 pays et territoires. La pandémie de Covid-19 a fait au moins 5.551.314 morts dans le monde depuis l’apparition du coronavirus fin 2019, selon un décompte établi jeudi par l’OMS.
